Jacob is currently filling his medication at his hospital specialty pharmacy for his chronic condition. He works closely with his Pharmacy Liaison, who recently renewed a prior authorization for his medication. Unfortunately, when the medication arrived, Jacob’s wife stored it in the freezer instead of the refrigerator, which ultimately rendered the medication unusable. When he spoke to his Liaison about this issue, she immediately jumped into action so that Jacob would not miss taking any doses of his medication. The Liaison coordinated with Jacob, the hospital specialty pharmacy, the Patient Assistance Program, the Medical Assistant and Jacob’s physician to secure a replacement for him. Fortunately for Jacob, his Liaison was able to obtain the medication from the manufacturer along with 3 additional samples in less than 24 hours. Jacob was extremely appreciative and impressed at how quickly his Liaison was able to rectify the situation for him, which would not have been possible if not for the Liaison’s role being embedded within the patient’s care team at the hospital. Because of his Liaison, Jacob did not have to miss a dose of his medication and was able to remain adherent to his treatment plan all along.
